III. Policy Statement
An enrolled student鈥檚 physical location for purposes of state authorization and professional licensure disclosures will be determined at the time of application to 妻友社区 College and at the time a student declares an academic major that leads to eligibility for licensure or certification when such declaration occurs after initial enrollment at 妻友社区 College. Students are responsible for notifying the College of any change in permanent and/or local address. Change of Address forms are available online through the Registrar鈥檚 Office (/registrar/).

IV. Definitions
Student Physical Location – means the U.S. state or territory of the student鈥檚 permanent (鈥渉ome鈥) address and residence at the time of Initial Enrollment, as self-reported by the student. After Initial Enrollment, for continuing or matriculated students, 鈥淪tudent Physical Location鈥 means the 鈥渃urrent鈥 or 鈥渃ampus鈥 address in which the student is physically present while engaging in coursework, clinical experiences, internships, or other required programmatic activities.
